‘There are pressures on the health system in terms of time and money. a pragmatic focus on outcomes for the patientīut while this approach is intrinsic to sound general practice, it is difficult to achieve a healthcare landscape that is leaving GPs feeling unsatisfied with the level of care, and time, they can give to patients.a thorough approach to information gathering. She says this comes down to ‘whole-person care’, the idea of looking at a patient as a whole person, rather than treating them as fragments. However, Dr Lynch believes promoting the ‘generalist philosophy’ can assist the ‘ongoing effort to help GPs value, hone and protect the often-misunderstood complexity of their work’. The college’s Health of the Nation report last year described general practice as being ‘in crisis’, with less than half of GPs surveyed intending to be practising in 10 years, and 73% saying they feel burnt out. ‘Understanding the sophisticated craft and practical skills of generalist care could help both GPs and health policy makers value, respect and navigate the challenges of this complex and important work, especially at this time of change for Australian general practice.’ ‘In order to protect GPs from doubting the value of their work, it is important to understand generalism as so much more than biomedical service provision: it requires philosophical attitudes, overarching priorities and practical skills that are often unnamed and unnoticed. ‘Clinical decision making in the face of uncertainty, undifferentiated symptoms, conflicted goals or chronic disease and grief is … cognitively, emotionally and morally difficult,’ they wrote. It is also why they are championing the ‘Craft of Generalism’, the acknowledgement of which is put forward as both a solution to poor healthcare policy and a way of improving morale among the general practice community. ‘Generalist care is undermined by systemic and bureaucratic devaluing of GP time and expertise, primary care policy that fragments care and encourages short transactional encounters, and an excessive focus on disease or procedures,’ they wrote. ‘There’s something that’s such a privilege in our practice, I have patients that I’ve seen for 25 years, but it also is demoralising when you continue to give and nobody’s noticing that you’re giving.’ĭr Lynch and her co-authors delve further into the issues in their article, explaining how Australia’s healthcare system is contributing to the problems, rather than helping to resolve them. ‘It’s really very gratifying, but it’s often done in a hurry and underpaid,’ she told newsGP. The lead author of a newly published article in the Australian Journal of General Practice told newsGP that their growing list of responsibilities now includes innovative care, political advocacy, financial and personal generosity, and countless examples of unpaid work – but without much additional support. The causes of these issues are complex and varied, but for University of Queensland Senior Lecturer and GP Dr Johanna Lynch, they can be partly explained by a healthcare system that has continually failed to understand the value and expertise of generalists. A prominent general practice academic says there is an urgent need to clarify the value and expertise of generalist approaches in healthcare.Ī greater understanding of the complexity of general practice could help health policy makers value and respect the challenges GPs face every day.ĭespite this year’s Federal Budget bringing some relief to a chronically underfunded general practice and primary healthcare system, it is no secret that many GPs are still struggling.īusiness sustainability and work-life balance remain chief concerns among the profession, while excessive red tape, workforce maldistribution, the lingering fallout from the COVID-19 pandemic and an ageing population are adding pressure on an already demanding job. In men, low testosterone levels lead to the accumulation of total and visceral fat mass, triggering a self-reinforcing cycle of complications that reduce the levels of this hormone. The mentioned effects of estrogen deficiency in the female sex can be eliminated by exogenous hormone replacement therapy 18, 20. In menopausal women, a decrease in estrogen levels increases total adipose tissue mass, reduces lean body mass 18 and results in the loss of subcutaneous fat with a simultaneous increase in its amount in the abdominal cavity 19. Moreover, it has been shown that binge eating is especially prevalent in the female sex and develops most often during adolescence, in combination with higher levels of estrogens 17. They cause weight gain, mainly by inhibiting the hormonal activity of the thyroid gland and modulating the hypothalamus 16. Research shows that estrogens promote the accumulation of subcutaneous fat not only in women, but also in men 13, 14, 15. The ability to effectively accumulate fat is an evident biological advantage for women due to the periods of pregnancy and lactation, which require high energy expenditure of the body 12. According to researchers, the factor that allows women to store fat more efficiently is estrogen. Women have more total fat which is deposited subcutaneously and in the gluteal/femoral regions, while men on average have less total body fat but more in the central/abdominal region 11. It is believed that gender-specific physiological mechanisms are responsible for the higher incidence of obesity in women than in men. Overweight and obesity arise as a result of excessive development of the fat mass in proportion to the fat-free body mass. Additionally, the frequency of this disease is gradually increasing 10. Obesity is a health problem with an epidemic status and the incidence of 15% for women and 11% for men 13% in the general population 10. On the other hand, on the basis of studies carried out in children aged 7–13 and repeated again after 4 years, slight but statistically significant changes of this indicator occurring in both sexes with age were less pronounced for the right hand, which is considered to be more sensitive to the influence of prenatal steroids 9. In other studies a significant increase in the mean 2D:4D values of the left hand was found in girls and boys between the ages of one and nine, while in the older age groups the changes turned out to be much smaller 6. Some authors did not demonstrate such changes 7, 8. So far, it has not been clearly established whether 2D:4D changes with age in humans. It has been estimated that the sexual dimorphism index for 2D:4D in adults is less than 5% 6. It is known that PT stimulates, while PE inhibits the growth of the fourth finger, therefore PT lowers and PE increases the size of 2D:4D and, as a result, it is on average lower in men than in women 2. Studies conducted in men also showed that genetically determined sensitivity to testosterone (polymorphism of the androgen receptor gene) is related to the digit ratio 5. Moreover, a negative correlation between the PT/PE ratio in amniotic waters in pregnant women and 2D:4D ratio in children has been demonstrated 3, 4. The influence of prenatal testosterone (PT) and prenatal estrogens (PE) on digit ratio supported in experimental studies conducted in mice 2. This indicator is determined in the prenatal period (between the 10th and 13th week of fetal life) under the influence of sex hormones 1. The 2D:4D ratio is the ratio of the length of the second (2D) to fourth (4D) finger of the hand. The rate of weight loss after bariatric surgery is independent of prenatal exposure to sex hormones. Moreover, it seems that the 2D:4D index (especially of the right hand) may be a useful factor in early prediction of the risk of developing excessive body weight in humans. This relationship indicates a new area of activity in the field of obesity prevention. We have found that the development of excessive body weight in men and women, and fat accumulation in the upper arms, thighs and lower legs in women with obesity (but not men) are associated with increased prenatal estrogen exposure. We tested 125 adults with obesity (BMI ≥ 30.0 kg/m 2), 125 adults with overweight (BMI 25.0–29.9 kg/m 2) and 153 persons with normal body weight (BMI < 25 kg/m 2) of both sexes. In the current study we investigated whether the 2D:4D index, as a marker of the prenatal hormonal environment, is associated with the development of overweight and obesity in adults, and whether is it correlated with the rate of weight loss in patients after bariatric surgery. Prenatal testosterone decreases and prenatal estrogens increase this index. 2D:4D finger length ratio is a proxy of prenatal sex hormone exposure. From that moment on, the children of Israel would look to God and God alone. By the time the captives of Israel returned to Jerusalem, the people had been cured of their idolatry. The captivity and exile of Israel were all part of God’s plan for His people, who He had chosen to discipline because of His love. This was the very idea behind Jeremiah’s most quoted verse: “’for I know the plans I have for you,’ declares the Lord, ‘plans to prosper you and not to harm you, plans to give you hope and a future.’” ( Jeremiah 29:11)Īs it was written in the Proverbs, “my son, do not reject the discipline of the Lord or loathe His reproof, for whom the Lord loves He reproves, even as a father corrects the son in whom he delights.” ( Proverbs 3:11-12) It would take 70 years in captivity for God’s message to finally get through and the hearts of Israel to return to God. Unfortunately, the people of Israel did not heed the warning and were taken captive by the Babylonians as a result. Among this first group of captives was the prophet Daniel, who resided in the palaces of Babylon and ministered to the king himself, while Ezekiel lived in the countryside.įor years, God warned the people of Israel and Judah, through prophets such as Isaiah and Jeremiah, that if they didn’t turn from their idolatry, He would discipline them for their disobedience and unfaithfulness. In fact, Nebuchadnezzar had previously carried off a number of high-level Jewish hostages in his first invasion of Judah in 605 B.C. and warned of the eventual destruction of Jerusalem at the hands of Babylon in 586 B.C.Įzekiel himself wrote most of his message from captivity as one of 10,000 Jews taken by King Nebuchadnezzar in 597 B.C. The book of Ezekiel was written by the prophet Ezekiel, who had ministered to God’s people from 592 B.C. Photo Credit: ©Sparrowstock Understanding the Context of the Book of Ezekiel It’s safe to say that Pulp Fiction definitely took some liberties.īut what does the real, biblical Ezekiel 25:17 actually mean? Why is the Pulp Fiction version so different?Īnd why is so important for believers to know what the Bible really has to say? I will execute great vengeance on them with wrathful rebukes and they will know that I am the Lord when I lay My vengeance on them. Blessed is he who, in the name of charity and good will, shepherds the weak through the valley of the darkness, for he is truly his brother’s keeper and the finder of lost children and I will strike down upon thee with great vengeance and furious anger those who attempt to poison and destroy my brothers and you will know I am the Lord when I lay my vengeance upon you!” “The path of the righteous man is beset on all sides by the inequities of the selfish and the tyranny of evil men. According to Jules, he’s been saying it for years. In one of Quentin Tarantino’s first feature films, the character Jules Winfield recites a modified version of Ezekiel 25:17 right before he shoots and kills one of his intended targets. Jackson, in Quentin Tarantino’s 1994 cult classic, Pulp Fiction. That’s because a version of Ezekiel 25:17 is famously quoted by hitman Jules Winfield, played by Samuel L. Ironically, it’s the movie buffs that have committed Ezekiel 25:17 to memory and elevated it to the top of most online search engines. Short, iconic, and easy to memorize, these are just a few foundational verses to the Christian faith that are often seen printed on bumper stickers or t-shirts or showcased on Twitter feeds.Įzekiel 25:17, however, is one verse that most Christ-followers and Bible readers may draw a blank on and yet, it’s one of the most popular and highly searched Bible verses in pop culture. “For God so loved the world that he gave his one and only Son, that whoever believes in him shall not perish but have eternal life.” ( John 3:16) “Trust in the Lord with all your heart and lean not on your own understanding in all your ways submit to him, and he will make your paths straight.” ( Proverbs 3:5-6) “for all have sinned and fall short of the glory of God.” ( Romans 3:23) “I can do all this through him who gives me strength.” ( Philippians 4:13) There are a handful of Bible verses that most Christians probably know by heart. “I will execute great vengeance on them with wrathful rebukes and they will know that I am the Lord when I lay My vengeance on them.” ( Ezekiel 25:17 NASB Translation) The other rather civilized aspect is that you can make slides using the trial version, and they will still work even after the trial expires, as the application makes its fonts available to the system.
